ICD-10 Code for Suicidal Ideation

ICD-10 Code for Suicidal Ideation: R45.851 Coding Guide

The ICD-10-CM code for suicidal ideation is R45.851, officially described as “Suicidal ideation.” It identifies suicidal thoughts documented by the clinician.

A client tells you during a session that they have been thinking about ending their life.  You assess suicide risk, decide on appropriate care, and document your findings. The diagnosis code should reflect that assessment.

This guide explains when to use R45.851, how it differs from attempt and history codes, and what documentation supports accurate reporting.

What Is the ICD-10-CM Code for Suicidal Ideation?

R45.851 falls under R45, symptoms and signs involving emotional state. It is a complete, billable diagnosis code and requires no additional characters. Billable status does not guarantee coverage or payment. R45.850 identifies homicidal ideations and should not be confused with R45.851.

What Does R45.851 Cover?

R45.851 falls under symptoms and signs involving emotional state. It is a complete, billable code; no additional characters are required. Billable status does not guarantee payment.

The same code applies to documented passive or active suicidal ideation:

  • Passive ideation: A wish to be dead or not wake up, without thoughts of taking action to end one’s life.
  • Active ideation: Thoughts of taking action to end one’s life, with or without a specific plan.

These descriptions do not determine risk level by themselves. Document the clinician’s assessment rather than inferring a diagnosis from an ambiguous statement.

Suicidal Ideation vs. Attempt and History Codes

Current ideation, a suicide attempt, and past suicidal behavior require different coding decisions.

CodeDescriptionWhen to review it
R45.851Suicidal ideationsCurrent suicidal ideation is documented
T14.91XASuicide attempt, initial encounterActive treatment of an attempt, when T14.91 applies
R45.88Nonsuicidal self-harmCurrent self-harm is documented without suicidal intent
Z91.51Personal history of suicidal behaviorPast suicidal behavior is relevant to current care
Z91.52Personal history of nonsuicidal self-harmPast nonsuicidal self-harm is relevant to current care

T14.91 is not the default for every suicide attempt. Review the documented injury or poisoning and the current Tabular List before selecting the code.

T14.91XA should not replace a more appropriate specific code. “Initial encounter” refers to active treatment, not necessarily the first appointment.

When Can R45.851 Be the First-Listed Diagnosis?

In outpatient care, R45.851 may be first-listed when suicidal ideation is chiefly responsible for the services provided and no related definitive diagnosis has been established.

When reporting a related definitive diagnosis and a separately reportable symptom under the Chapter 18 rule, list the definitive diagnosis first.

The R45.85 category has an Excludes1 note for suicide attempt (T14.91). Do not report the excluded codes together when they describe related conditions. If the relationship is unclear, seek clarification.

Practical Coding Examples

Current Ideation Without a Related Confirmed Diagnosis

A client reports current suicidal thoughts. The clinician assesses the presentation and documents suicidal ideation without establishing a related definitive diagnosis. Review R45.851 as the reason for the outpatient encounter.

Past Attempt With Current Ideation

A client has current suicidal ideation and a remote suicide attempt relevant to care. Review R45.851 for the current finding and Z91.51 for the documented history. The history code does not replace the current symptom.

Past Behavior Without Current Ideation

The clinician documents a previous attempt but does not diagnose current suicidal ideation. The history may support Z91.51; the past event alone does not support R45.851 today.

These examples illustrate code distinctions; final reporting depends on the complete record and applicable instructions.

What Documentation Supports R45.851?

The note should explain the findings, assessment, and care provided. Use these prompts as appropriate to the encounter:

  • Current thoughts and passive or active ideation
  • Findings about plan, intent, access to means, and behavior
  • Relevant history, risk factors, and protective factors
  • Clinical risk assessment and supporting rationale
  • Selected interventions and care setting
  • Safety planning and review, when appropriate
  • Follow-up arrangements and patient instructions

A structured tool can support assessment, but its score does not replace clinical judgment. SAMHSA’s SAFE-T framework offers guidance for assessing risk and documenting the response.

Which CPT Codes May Apply?

Choose the procedure code for the service provided, not simply because R45.851 is documented. Depending on the service and eligible practitioner, relevant codes include:

  • 90791: Psychiatric diagnostic evaluation
  • 90792: Psychiatric diagnostic evaluation with medical services
  • 90832, 90834, 90837: Time-based individual psychotherapy
  • 90839 and 90840: Crisis psychotherapy; 90840 is an add-on to 90839

Suicidal ideation alone does not establish that crisis psychotherapy was provided. Document the presenting crisis, assessment, interventions, disposition, and applicable service time. Apply the current service-specific time, documentation, and code-combination rules before billing.

What Should Telehealth Documentation Address?

Confirm the address where the client is physically located during the visit and a working callback number. Maintain an emergency plan covering:

  • Local emergency-service numbers and escalation arrangements
  • A nearby support person, with appropriate authorization
  • What to do if the connection fails during a crisis

Record other people present when relevant to privacy or assessment. Separately, verify coverage, place-of-service codes, and modifiers for the billed service.

Coding Errors to Check Before Billing

Compare the claim with the encounter documentation. Check for:

  • Copied assessment language that does not reflect the visit
  • Conflicting statements about ideation
  • Attempt and ideation codes used interchangeably
  • Procedure codes unsupported by the service or time

Clarify conflicting notes rather than making assumptions. A denial of ideation later in an encounter does not necessarily negate earlier documented ideation.

Use the code set effective for the applicable encounter or discharge date, including annual October updates and applicable April updates.

Keep Behavioral Health Notes and Billing Connected With Vozo EHR

When notes, appointments, and billing sit in separate systems, your team has more places to check before completing the encounter.

Vozo EHR brings clinical documentation, scheduling, telehealth, and billing into one platform. Its template tools let practices create or adapt encounter notes around their documentation needs.

With Vozo, your team can:

  • Build and customize templates for encounter documentation
  • Manage appointments and telehealth within the same platform
  • Connect clinical documentation with coding and claims workflows
  • Keep patient communication alongside daily practice operations

Review how your team would document sessions and manage billing in practice.

Frequently Asked Questions

Is R45.851 a billable ICD-10-CM code?

Yes. R45.851 is complete and does not require additional characters. Payment depends on the documented service, coverage, and applicable payer requirements.

Can I use R45.851 for passive suicidal ideation?

Yes, when the clinician documents passive suicidal ideation. Record the client’s reported thoughts and the assessment. Passive and active ideation do not have separate R45.851 subcodes.

What is the difference between R45.851 and T14.91XA?

R45.851 identifies suicidal ideations. T14.91XA identifies active treatment of a suicide attempt when that code applies. Review injury or poisoning instructions and the R45.85 Excludes1 note before reporting attempt-related codes.

Can R45.851 be the first-listed outpatient diagnosis?

It may be when suicidal ideation is chiefly responsible for the services provided and no related definitive diagnosis is established. Follow the encounter-specific sequencing rules.

Can R45.851 be reported during a telehealth encounter?

R45.851 may be reported for documented suicidal ideation during a telehealth encounter. Reimbursement depends separately on service coverage and applicable telehealth billing requirements.
